Online appointment booking is managed through GoRendez-Vous, a Quebec-based platform. The information collected for this purpose includes:

• First and last name

• Email address and phone number

• Information required to manage your appointment

3. Tools Used in Administrative Practice

Notion

I use Notion for the administrative management of my practice. Certain personal information (name and email address) may be recorded for administrative tracking purposes. Notion is a platform whose servers are located outside of Quebec, in the United States. To view their privacy policy: https://www.notion.so/privacy 

Gmail (via Notion Mail)

Email communications are managed through Gmail, a service provided by Google LLC, accessed via the Notion Mail interface. Emails exchanged may contain personal information such as your name and email address. This data is processed on servers located outside of Quebec. Claude (Anthropic) — Writing Assistance

I occasionally use Claude, an artificial intelligence tool developed by Anthropic, for writing and administrative assistance purposes. No personal information about you is shared with this tool. Its use within my practice is strictly anonymous.

7. Transfer of Information Outside Quebec

Some tools used in my practice (Notion and Gmail) involve the processing of data outside of Quebec. Prior to using these services, I assessed that the protection measures in place offered an adequate level of protection, in accordance with the requirements of Law 25. Appointment booking through GoRendez-Vous is hosted in Quebec.

 

8. Retention Period

Information collected via the contact form is retained for the duration necessary to process your request, then deleted. Administrative information is retained in accordance with the legal obligations applicable to healthcare professionals in Quebec.

 

9. Security of Your Information

I take reasonable security measures to protect your personal information against loss, theft, misuse, unauthorized disclosure, unauthorized access, modification, or destruction.

 

10. Your Rights

In accordance with Law 25, you have the following rights:

• Right of access: View the information I hold about you.

• Right of rectification: Request the correction of inaccurate or incomplete information.

• Right to erasure: Request the deletion of your information, under certain circumstances.

• Right to portability: Receive your information in a common technological format, or request its transfer.

• Right to de-indexing: Request that a hyperlink associated with your name be de-indexed if its publication causes you harm.

• Right to object: Object to the processing of your information in certain cases.
